Professionals can initiate a referral by faxing their referral form to 226-7508 or by calling us at 228-0547 and pressing option 3 to connect. It is helpful to be sure to include a name and contact information for the child's current caregiver and to clarify who the legal guardian of the child is, if different from the current caregiver. The process includes an initial phone screening typically lasting approximately 20 minutes, review of the referral by a clinician, gathering of our intake paperwork packet and any other relevant information we need (such as parenting plans, guardianship certificates, or restraining orders), and, once all is gathered, we will outreach for scheduling either in office or via telehealth at the family's preference.
