Connecting patients with the right behavioral health resources can be a challenge. Follow the steps below to help ensure a quality behavioral health referral:
-
Screening & Assessments: Use evidence-based screening and assessment tools to clarify the level of care and determine what services may be needed.
-
Shared Goals: Join your patient in treatment planning. Determine what they are motivated to work on and what types of services they are willing to consider.
-
Identify Resources: Identify providers who offer the services your patient needs. Be sure to review eligibility criteria such as age or insurance accepted before making the referral.
-
Closing the Loop: Check in with your patient and/or the referred provider to ensure goodness of fit before the referral is considered complete. Support coordination of care by sharing relevant documentation.
