The goals of this initiative are:

1) to increase the number of New Hampshire youth ages 12 - 22 who are screened in primary care settings and who receive clinical response and referral appropriate to their level of risk; and
2) to have youth and young adult Screening Brief Intervention and Referral to Treatment (SBIRT) adopted and fully supported as a standard practice within NH primary care health systems
