One of six learning collaboratives in six states and the District of Columbia undertaking implementation of improved, safe, and seamless transitions of care from pediatric primary and special providers to adult health care providers using the Six Core Elements of Health Care Transition developed by GotTransition.
Practice setting
Most practice teams were successful in developing a written health care transition policy, a registry of transition-age youth and young adults, use of a transition readiness assessment, use of portable medical summaries and transition plans, and checklists for the process of handoff of care from pediatric setting to adult setting.
